Who can apply for this card?

Anyone at the age of 21 years and over and has a minimum annual income of RM24,000 or RM2,000 monthly can apply for a Lazada UOB Card.

What are the required documents to apply for a Lazada UOB Card?

For Malaysians, please prepare the following documents to make an application.

Salaried employee:

• Copy of your IC (front and back)
• Latest 3 months' salary slips

And any of the following documents:

• Latest Form B/BE (supported with tax payment receipt)
• Latest EPF statement
• Latest 3 months' bank statements
• Latest BE Form with a tax receipt

Self-employed:

• Copy of your IC (front and back)
• Photocopy of Business Registration Form
• Latest BE Form with a tax receipt
• Latest 6 months' bank statement (showing business turnover)

For foreigners, please prepare the documents below.

Salaried employee:

• Copy of Passport and
• Latest 2 months' salary slip
• Work permit valid for at least one (1) year at the time of application and
• Letter of employment

What is the annual fee for the Lazada UOB Card?

The annual fee for the principal card is RM100 and RM70 for the supplementary card. You can enjoy an annual fee waiver simply by spending RM15,000 and above yearly.
